The Antimicrobial Stewardship Pharmacist is responsible for assisting with the clinical activities and providing medication information for the hospital and also development, implementation, and documentation of the antimicrobial stewardship program for West Jefferson Medical Center. This will be accomplished through collaboration, coordination and communication with the Clinical and Staff Pharmacy Team, medical staff, leadership members and other healthcare workers. This pharmacist will coordinate the pharmacokinetics service according to the most recent guidelines and recommendations published in pharmacy and medical literature. This pharmacist will assist other hospital personnel in the development of clinical pathways, protocols, policies and procedures and provide advanced education to students, pharmacists, and other health care professionals. The job duties are completed by utilization of professional pharmaceutical knowledge, judgement, policy, procedure, skills, standard controls, and ethics.

 

Licensure(s): Current Pharmacist License Required

Registration(s): Registered as a Pharmacist by the Louisiana Board of Pharmacy Required

Preferred Related Experience (1-2 year): PGY1 hospital residency training or 5+ years of pharmacy experience in acute care setting or BCPS certification

Preferred: Doctor of Pharmacy (Pharm.D.)*
